TEACHER ACTION PLAN (TAP)

1.       Document Analysis

           A.       Review credentials i.e. transcripts, certificate, prior experience.

           B.       Review self assessment outline.

           C.       Summarize date.

 2.       Observation

           A.      Determine if an observation is necessary prior to development of TAP.

           B.       Conduct observation if needed.

3.       Write Teacher Action Plan as a Team

           A.      Include timeline.

           B.       Include specific goals for improvement.

          C.       Show steps you will take to achieve goals.

           D.      Show how goals will be evaluated.

           E.       Type, sign, and submit TAP to SSSC before October 20.
