Please complete and return to your child's school before Sept 30 if you would like your child to participate in our dental clinic. If you cannot print this form at home, please call the school and we will be happy to mail you a paper copy. Smile for Life Dental Clinic dates will be in October and March this year (exact dates TBD).
If remote learning students would like to take part in our dental clinic, complete the form and return it to school. Write REMOTE STUDENT somewhere on the form and we will be in touch with you to schedule your dental clinic time.
